If a Visa is necessary, it is the B1 - Tourist, correct? (The one for 500Rp)
Visa is needed (at least as of last month). If you have enough time (at least 2 weeks) you can apply for the eVisa (e-VOA) from here, but VoA would be the easiest way to go. Correct, it's the B1 visa which costs 35USD or 500K Rp (not 500Rp) per person. Note that you have to pay in USD or Rp only at the VoA counter (cards not accepted). There are multiple ATMs near the counters if you want to withdraw Rp using your Debit card.

What is the current status of the Visa / VoA requirement for Indians? Some websites claim it is not necessary if the stay is less than 30 days. I see this has been asked before; just curious how Tata AIG / HDFC Ergo have incorrect info on their website.

Yes, Visa is required and VoA is the best option.

It's not cash only; you can use your credit card. I paid using my credit card
