A manufacturing process be it any sector - Automobiles, FMCG, has process development along with product development as a part.

Now any process in the manufacturing of the car along with the parts used in it are manufactured with an SOP formed by the manufacturing team. As time passes and technologies advance, these SOP's change/revise updating the process so as to save time, energy and resource utilization.

As for your question as to what could have changed after 2011 is something that would have been revealed during their R&D work on the parts used. What testing was done to approve the product would have been initially but as times passed, the material could not withstand the abuse hence gives way making it a customer complaint.

The R&D team keeps updating the process, materials to be used, SOP to manufacture but this is not compulsory that all vendors keep the R&D team in the loop to inform then about any change in specifications of the materials. The vendor could have changed the part silently but due to repeated complaints Chevrolet would have been forced to investigate and henceforth the recall. The other case could be, Chevrolet could have known the fault but was silent till the water reached the head.

Such recalls are not one-man initiation and execution., it needs many hands, brains and time to get in the right manner so to avoid any future complaints.

Absolutely agree with you anurag. It is indeed a great loss to the manufacturer. Especially in India, outside forums like Team-BHP, recalls means major issues with the design of the car and affects the reputation badly. But i do not agree to silent recalls like these without informing the public as to what is changed and why.

The customer has every authority to know what is being changed and for what along with that what if the part is not changed/ignored along with the outcome.
